The Family Group Sheet 

One of the first documents you'll start recording your family history with is the "Family Group Sheet." On this form you'll begin your genealogy research by recording:

• Your full name
• Your birth date
• Your birth place
• Your marriage date (if married)
• Your spouse's full name (if any)
• Your spouse's birth date
• Your spouse's birth place
• Your spouse's death date (if any)
• Your spouse's death place
• All the same information about your children
